NRCan Photo Identification Renewal
To Candidates with a Qualified Operators / Certified Exposure Device Operators Ticket:
It is important to note that the Canadian Nuclear Safety Commission (formerly Atomic Energy Control Board) does not consider your certification valid unless your Qualified Operator/Certified Exposure Device Operator ticket is accompanied with a photo identification card issued by the NRCan National Non-Destructive Testing Certification Body, bearing the same registration number.
Applying for renewal
To apply for renewal of your NRCan photo identification card, complete the application form enclosed with this notice and return with two (2) passport photographs, and a $120. renewal fee to the NDT Certification Body.
Completing the photo identification renewal application form
Part 1 is the identification card photo verification. This section is to be completed and signed by a witness. The witness could be the applicant's supervisor, a bank employee, doctor or other professional person.
Part 2 is the type of payment. Identify your payment as a cheque, money order, MasterCard or Visa. Personal cheques are not accepted for renewal. When paying by credit card, it is important to print the cardholder's name, card number, amount and expiry date. Cheques and money orders are to be made payable to the Receiver General for Canada.
Renewal fee
The renewal fee for the up-coming six (6) year term will be $120.00.
